# Care plan review

A care plan review is a scheduled check of whether a care plan still fits a person's needs. The family, caregivers and others look at what works and what does not. They adjust tasks, hours or services. Reviews also happen after a health change.

## Why it matters for your family

Needs change faster than paper. A review after a fall, a new diagnosis, a hospital stay or a few weeks of care can reveal gaps. Ask what has changed in mobility, memory, sleep, appetite and mood. Check whether the hours still match the need. The National Institute on Aging says care managers help form a care plan, and families may update it as needs shift. A common mistake is to wait for a crisis. Set a regular date, such as every few months, and add reviews after events. Include the person receiving care. Record changes in writing and share the new version with every caregiver.
