# Daily care notes

Daily care notes are short written updates a caregiver leaves after each shift. They describe tasks done, how the person felt and anything the next caregiver or family should know. They are part of the care log. Good notes are factual, dated and easy to read at a glance.

## Why it matters for your family

Notes bridge shifts. A caregiver who leaves at 5 p.m. can tell the evening caregiver that the person skipped lunch, felt dizzy or had a good walk. Families can read notes without calling. The National Institute on Aging offers caregiver worksheets for coordinating care. Keep notes simple: date, times, meals, activity, mood, concerns and supplies needed. A common mistake is to write vague lines such as "fine." Another is to omit small changes that signal a bigger problem, like less appetite. Write only what you saw. Share urgent concerns by phone, not only in a note. Store notes where all caregivers and the family can read them.
