Commonwealth Coordinated Care Plus (CCC Plus) Waiver (Virginia)
The Commonwealth Coordinated Care Plus Waiver is a Virginia Medicaid waiver that pays for care at home for older adults, people with physical disabilities, and people who are chronically ill and need ongoing skilled nursing care. The Department of Medical Assistance Services runs it. A person must also be eligible for Medicaid.
Why it matters for your family
A Community Based Screening Team, made up of a social worker and a nurse, assesses need. A person can start through the local Department of Social Services, then apply for Medicaid online, by phone, or on paper. Services include personal care and respite, which can be agency-directed or consumer-directed. DMAS pages do not mention a waiting list. A common mistake is to apply for Medicaid first and skip the screening. Both steps are needed. Ask your local Department of Social Services to request the screening. Families can also ask about consumer-directed options, where the person helps choose and supervise the worker.
