PASSPORT Medicaid waiver (Ohio)

PASSPORT is an Ohio Medicaid waiver that helps older adults get care at home instead of in a nursing home. The Ohio Department of Aging runs it with the Ohio Department of Medicaid. Applicants must be age 60 or older, meet Medicaid financial rules, and meet a nursing facility level of care.

Why it matters for your family

PASSPORT services include adult day health, homemaker help, home-delivered meals, and home care attendant services. Local PASSPORT administrative agencies help with enrollment. A person asks for the waiver by submitting form ODM 02399 to the county Department of Job and Family Services. If no slot is open, a waiting list may apply. A common mistake is to think the waiver is open to people under 60. Other Ohio programs serve younger adults. The state may seek repayment from the estate for some costs. Call your local Area Agency on Aging to start.
