Texas Primary Home Care

Texas Primary Home Care, or PHC, is a Medicaid-funded personal care program run through the Texas Health and Human Services Commission. It gives non-skilled help with daily activities to people with full Medicaid whose chronic health problems limit daily activities. A practitioner documents the medical need on Form 3052.

Why it matters for your family

Attendants help with tasks such as bathing, grooming, meal preparation, and housekeeping. PHC excludes skilled tasks, such as catheter insertion, tube feedings, injections, and medication administration. Agencies need a state license and a contract. Do not confuse it with the Primary Health Care program, which is a separate clinic program. A common mistake is to expect an attendant to handle medicines. Attendants may not give them. Ask HHSC or your health plan whether you qualify and how many hours the assessment approves. Do not count on a private agency to know your options, so ask HHSC directly as well.
